Attendance Slip

Attendance Slip is the form used in recording and verifying the attendance of University of the Philippines (UP) male students during Reserve Officers Training Corps (ROTC) training days. The attendance slips are usually distributed a few minutes before the end of each training day. All cadets must write the needed information (name, student number, UP ROTC unit, etc.) on the two sections of the attendance slip – the cadet's copy and the department's copy. The UP Department of Military Science and Tactics (UP DMST) office in the different UP units will use the department's copy to tally the attendance of the cadets. If there are any discrepancies in the records, the cadet's copy will be used as proof to indicate the presence of a cadet on a particular training day.
